What precautions should I take after the embryo transfer ?


One of the commonest questions patients ask me is - " Doctor, what are the dos and don'ts after the embryo transfer ? " They are very worried about the risk of failure, and are petrified that they may end up doing something which will cause their embryos to "fall out" of the uterus. Many will often spend the entire 2 week wait obsessing about their diet and physical activity.

Some will even put themselves on voluntary house arrest. Even worse, their relatives will often force them to sleep in bed for 14 days after the transfer. And to add insult to injury, many IVF doctors will also "advise" this !

Why do we still continue to perpetuate this myth ? I think it's partly doctors who are responsible For one thing, many doctors do not bother to educate their patients that physical activity will not affect embryo implantation. Others have a hidden agenda. Some want to hold on to their patients and by claiming that bed rest is essential after the IVF treatment, they can dissuade their patients from going to Bombay for their treatment ! Other doctors use this as a subtle way of " blaming the victim " if the cycle fails. ( " Your IVF cycle failed because you did not rest" is the hidden subtext !)

The reality is that bed rest does not help to improve pregnancy rates after IVF. Many studies have proven this conclusively. As I remind patients, the human body has been designed with enough sense, that embryos do not fall out if your cough or sneeze ! The truth is that human reproduction is inefficient and we cannot control the outcome. Whether the embryo implants or not is a biological process which the patient cannot influence in any way !

Am I taking an excessively casual approach ? No, I am just taking a realistic approach ! The problem is that when patients do not get pregnant, they often end up blaming themselves - and a common regret is - I did not rest enough. And even if they are smart enough not to do so, there are enough "friends and well-wishers" who will do this for them ! " What , you did not rest ? Your doctor must be crazy giving you such bad advise ! When my sister did her IVF in Delhi, her doctor made her rest for 9 months and now she has a beautiful 2 year old daughter" !

Once the embryos have been transferred into the uterus, they are safe and cannot "fall out " . This is simple commonsense ! IVF embryos , once they have reached the uterus, do not "remember" that they have spent 3 days in an IVF incubator - they are just like all other "in vivo" embryos which are formed after intercourse in the bedroom. Whether the embryo spends 3 days in your fallopian tube or in a test tube makes no difference to the outcome !

You need to have enough maturity to understand that the final outcome is not in your hands. It's important that you not blame yourself in case you do not get pregnant - doing this just makes a bad situation worse ! Trust your gut instinct and your commonsense - your body has enough wisdom to guide you as to what is fine and what is not. My advise is simple - Don't do anything which you may regret later !

Curtailing physical activity during the 2ww is a huge challenge for women who already have a baby and are doing IVF to complete their family. They feel torn between their desire to pick up their toddler and their fear that this "strain" may expel the embryo !

A word of advise to spouses. You are very likely to be overprotective during this time. You will want her to "rest" and "take things easy". You will tell her not to go to work, or to be very careful as to what she drinks or eats. While I am sure your wife will enjoy this pampering, please do not go over the top. She is smart enough to figure out what to do - after all, she is the one who has the most at stake, and is unlikely to do anything which will jeopardise her chances of getting pregnant ! Try to protect her from the well-meaning advise of busy-bodies, who are often very poorly informed !

Actually, this bed rest can be harmful; and we do our best to encourage our patients to go back to work after the embryo transfer ! Keeping yourself gainfully occupied is your best bet. Sitting at home when you are well can be very demoralising.

It can be very hard to deal with the uncertainty and anxiety of the 2 week wait ! The emotional stress is enormous - and lying in bed all day just exacerbates this a thousand fold ! Patients on bed rest tend to brood all day long about the outcome. Every minor twinge and ache is magnified, and your mind plays games with you during the 2 week wait - and commonsense suggests this stress cannot be good for you ! Moreover, when you are on bed rest, you are a burden to the rest of the family, and this adds to your guilt ! And if the cycle fails, then you end up blaming yourself ( I got up to pee after the embryo transfer, and this caused my embryos to get dislodged !)

You cannot influence the outcome - all you can do is gracefully let nature take its own course !

    a very helpful article.. I have told this to many people in my family thousand times including my wife. But somehow people tends to believe things that scare them more often than things that are true. I hope coming this from Dr. Malpani will help many people

      Dr Malpani,i am Dr Debarati Jha frm kolkata.this is my 3rd ivf done today becoz of endonetriosis.last time preg was pos -monochorionic twins.but prom occurd at 22 wks.my endom was 10.6mm wth zone 3 blood flow.as i am a doc i had to ho hosp but was doin light work.do u think i shud take complete bedrest this time and what r my chances of getting pregnant this time.hope to get ur ans at d earliest

    Sir, I really don't know u like the other here who see you as an authority. However, u made what people in my country refers to as "too much sense". I am Nigerian and I had my ET some days ago in my country, I was at work the next day and I just believe that God has made it very successful.     Thank you for the info. I really needed it this particular period of time. I had my FET transfer yesterday and I was not sure if I did something wrong last month that made the fresh transfer failed. Your article really helped me not to blame my self for the past and pray for God to help me that things would work for me this time....

    Hiya dr, i just had icsi transfer today and I'm resting up I was a bit worried about going to work tomorrow, but now I'm more calm about it. And I'll remember to use my common sence as my job is worki g with those that carnt help them selves, so lots of hoisting lifting ect. But I'll stay well away from all than and just do light dutys. Your prayer was lovely and insightfull, I'll keep it with me always x

    Dr. M I just had my FET done yesturday and the only thing I am stressing about is that I took ibuprofen 800mg 1x at 11 pm the night before. My transfer was at 9:45am. My RE didnt mention not to take it but when he said after not to take ibuprofen only Tylenol for cramps I said oh no I took some last night. He and the other doctor looked at each other and then were like I'm sure it's out of your system. But it looks like it stays in your system 24 hrs and can mess with implantation and early miscarriage.

  17. No, that is NOT true. Think about this for a minute. Women have sex during their fertile time, without even knowing they are ovulating. Many take Ibuprofen before and after ovulation, and \ does not have any effect on their fertility at all.

    Please stop beating up on yourself !

  19. There are no special precautions you need to take. Implantation is a natural biological process !

      I had my 5 day transfer 3 days ago and I feel no symptoms, I had mild ohss which seems to have settled. Does this mean I can't be pregnant? Ps. I have also been sneezing, coughing and laughing since transfer have I affected implantation?

    1. If vaginal progesteron 8% gel is a very good option why it is not being prescribed by most practioners?
    (It is a traumatising experience to take the injectable progesterone. cant bear them!!!)

    2. what is the role of Prednisolone after embryo transfer because I have been told to take it.

  21. We stopped using injectible progesterone 20 years ago !

    Prednisolone is a steroid and some doctors believe it helps embryo implantation ( by suppressing the immune system)

    I got bleeding on 12 th day post ET.(it was the day due for my period) my UPT is Also Neg. so it means implantation didnt occur?

  25. A negative UPT is not a good sign :(
    However, you should do a blood test for HCG as well - this is much more reliable.

    Yes, you can do a FET in the next cycle if you have bounced back

This have been going on since october 2012, i did pap smear test,my Dr said the result was ok. I did hormonal profile,and all they could see was that i am not ovulating.     This maybe because you have PCOD ( polycystic ovarian disease).

    You need the following medical tests.
    1. blood tests for you for the following reproductive hormones - FSH ( follicle-stimulating hormone),LH ( luteinising hormone),PRL ( prolactin) , AMH ( anti-Mullerian hormone) and TSH ( thyroid stimulating hormone)
    ( to check the quality of your eggs). Do this from a reliable lab such as SRL ( www.srl.in);
    2. a vaginal ultrasound scan which should check for the following.

    a. ovarian volume
    b. antral follicle count
    c. uterus morphology
    d. endometrial thickness and texture

    Please send me the detailed test results and medical reports . You can scan them in as a single Word file and email them to me.

    Once we know what the problem is, then we can treat it !

    Hi Dr, I had a day 3 ET on Monday and did acupuncture before& after and again on Thursday ( day 6). The acupuncturist was using heat packs to keep me warm during treatment. Is this safe? He insisted that it was an important part of the treatment as it helped to increase blood flow to the uterus & this is the main aim of acupuncture after transfer but I was worried that it may overheat the embryo. He also used gentle massage on my stomach and more vigorous on my lower back & again said it was safe for the embryo but I have been worried about it ever since and have many sharp twinges in my abdomen, some just from sitting up. Thanks for your help.

    I had done my et on 2nd day with 3 embryo
    1 is 4 cell
    2 and 3 is 2 cell

  52. Too early to do a urine test for 2 reasons.

    1. Even if you are pregnant, the urine test may not be positive, because the HCG levels in the urine are often very low at this time. This is called a false negative, and can depress you.

    2. Even if you are not pregnant, the urine test may be positive, because of the HCG present in your body as a result of the HCG trigger shot given before egg collection. This is called a false positive, and can create false hope.

    Please be patient - I know this waiting can be hard !

    Hi Dr, i am pregnant with IVF and i am 10wks today. I had two embryo transfer and my Dr said only one implanted.I started spotting 2days ago (brown and a little red on the tissue when i wipe), the Dr said also that it may be the one that did not implant that is coming out gradually but i should not worry. Dr, i am scared and i hope all is well. I did a urine test and it showed two lines that means i am still pregnant.Dr what is your opinion? Thank you.

    I will using a puregon injection this time after 10 days I will be scheduled for embryo transfer. I am happy that I can still work after embryo transfer.. I did rest during my first ivf but it was not successful. I worried because I do lots of moving in my work as I am in catering department delivering food to the patients..pushing the trolley to the wards.. is it safe to work after embry transfer.. from lucky woman

    Thanks. doctor. This is quite encouraging. I had my transfer done on Thursday . It was a day 6 day embryo after icsi and assisted hatching.
    I feel headaches all morning up till now. With cold feet and hands. But otherwise I am fine. Also a bit concerned what I should eat. Best diet for now.

  62. Hi Doc, I have thyroid (on 50 mcg Thyronorm), PCOD, Endometriosis and Rt. Fallopian tube is blocked. I underwent Laproscopy in Jan'13. I just came to know that my first ICSI cycle has not been successful. Though I had 9 eggs and 3 gud embryos transferred, it was not successful. I am shattered by the news, as this was our only hope. Could you pls guide us .. Thanks

    I just had embryo transfer done this afternoon. I still have aches from the eggs retrieval 2 days ago. I had a difficult time passing motion and used quite a lot of strength to push. Now I suddenly noticed a small blood clot under my belly button...I'm not sure if it was there since afternoon or now around 830pm. what could it be please? there is no pain .. I am worried. ..

    Thanks Doc for your write up is very educative I had three fail treatment two in my home country Nigeria while the third one was in New Delhi after which I had a lasprocopy surgery to take off uterine fibroid . Had to use egg donor during my third treatment . My question now is are we suppose to stay off certain food to increase our chances of getting pregnant

  66. The laparoscopic surgery to remove the fibroid was a bad idea.

    Intramural fibroids ( in the wall of the uterus) do not need to be removed as they do not affect embryo implantation.

    Only submucous fibroids ( which are in the uterine cavity) need to be removed. These can be best removed with an
    operative hysteroscopy ( www.drmalpani.com/hysteroscopy.htm) to normalise the uterine cavity.

    hello doc, recently i have done IVF i got positive in home pergency test card but, after 10 days my doctor asked me to come for scan.
    SCAN RESULT is
    Thick endometrium 10mm
    No free fluid
    couldnt see the embryo.

    next day again checked for home pregency test it came negative result. My doctor asked me to repeat the Ivf process after 3 to 6 months. can you help me to understand why this is happend. what is the problem Doctor?

    Hi Doctor,

    I had my 3rd ET on 18th of this month. Today is my 6th day. I had 2 failed ET before this one. This time I had 2 embryo transfer. One is 8 cells and another is 4 cells. After the ET I caught cold and dry cough and I'm having to cough too much which giving me quite bad pressure on my body.And also I'm having dry cough syrup. Is it gonna effect to my embryo in my uterus not to be pregnant.

    Normally how soon you can feel any symptom for pregnancy after ET. I was told to do the test after 2weeks but I don't really feel any symptom. Really disappointed and dad about that. I know it's early to say but still just worried because of the 1st two failed. Anything I can do to make it work. please help.

  72. Hi, I am 31 year and have went through the 1st cycle of IVF as diagnosis says bilateral endometriotic ovarian cysts left and right with bilateral tubal adhesion & pelvic adhesion. So my last month i went through IVF cycle but the beta hcg was 27.3. Doc suggest to stop all medicine and wait to get my period. May be she will plan next month. kindly sugg

Dear doctor, yesterday I did my FET around 2pm at Kolkata since I am not from this place I have to travel back home by flight for 1hr and by road for 4hrs...will that effect my FET bcos I am leaving in 3 days after FET...and one more thing is that after doing my FET I passed urine heavily because I've been drinking water and i have been holding it since the past 3hrs...will that effect my FET?     I really appreciate your advice and your article was quite informative and help me alot to calm down.
    today is 6 day of my ET, one embryo was transferred which was morula but what is worrying me that am having pains like what i use to have before periods so is it normal? secondly i work i took a week rest and will be going on job from tomorrow, i live on first floor so climbing stairs is safe after ET ? Please reply bez these pains are a big cause of depression and tension for me. Thanks

    Hello dr ive been treated for stage 4 endometriosis in the month of aug from then 3 doses of luprid for an interval of 28 days. My normal flow of periods started in dec bt there were no folicals n one simple cyst on rt overi. My next cycle I had two follical 21 mm on right n 17mm on left . my husband's counts r 10 mml. I ve gone through IUI . I wan to knw wat r the chances of conceiving after my endometriosis surgery n with my husband's count. Planning for IVF for nxt cycle wat is advisable.

I had seven embryos retrieved out of which one was of grade a and 3 were of grade b and rest were of grade c my Frozen BET is scheduled for tomorrow should I take stairs as our we live at third floor. Today I have to step out and be at a work event making me be in the car for an hour each way and another hour at the event.     Hi there, I did a d3 ET today. I went to my acupuncturist a few hours afterward, she placed heat lamps over my feet and abdomen and left me for at least 30mins. When she returned I was extremely hot. I was unaware that I shouldn't be exposed to heat. Could this have harmed my 2 embryos?

    Greetings for the Day !!

    Have done IVF and it is my third month, irrespective of the health condition doctors are mandating all the patince to stay there for a minimum duration of 5 months is it necessary. Also as a common practice they do stitching for all the patience who had sucessfull IVF after 3 months. Is stitching mandatory also do we need to have any special injection via trips after stitching this is also common to all patience, hence they are mandating us to stay for a minimum duration of 5 months. Kindly suggest

    Hi Dr.
    I did 5dt on March 21(Monday), I rested 1 day and I went to work on Wednesday and Thursday. The school I teach is a story building and I had to climb like 2 or 3 times daily. Can this affect my chance. Also today since around 3hrs ago, I've been having noisy stomach with little pain, though no bleeding/spotting nor colored discharge just whitish . Thirdly, anytime I insert the progesterone thru vagina, it starts coming down like 15 mins afterwards. Is it normal? Fourtly, I see the whitish discharge as the color of the progesterone, am I right?

    I m 29 yrs young.I had pcod. No follicles with clomiph and gonadotrophins.laproscopy done, normal uterus and mildly dilated tubes. Ovarian drilling done, followed by 3 failed IUI. Opted for IVF. 26 eggs retrieved, 8 blasts came. Since I developed ohss ET was planned 1 month after with 2 XB blastocyst.my endo thickness was 9mm at the time of transfer.all my blood profiles were normal including E2, P4. I was put on bed rest during the 2ww.went home on the same day of transfer.I was put under metformin,estrodiol, vaginal progesterone and supplements, after with all these my cycle failed. Wat could be d reason sir?? M frustrated a lot.kindly help me to make the second cycle a successful one. Thank you dr

    Hello Dr. Tommorow is my day 3 transfer. Blastocyst was not recommended as got only 5 embryos also report say Remark as egg fragile and granular. What does it mean? Now for success how many embryo to transfer. Require ur precious suggestion.Thanks in advance

    Hello doc. My wife had ET three days back with two compacting embryos out of five that were freezed for two months. Laser assisted hatching was done. Endo is 10mm. Now, on Geston tablet, 2mg estrabet thrice daily, wysolon and ecosprin. Fingers crossed.

    Hello doctor,
    I am 26 years old we underwent ivf today 3 fresh embryos are transferred.( treatment is done in my native place) .we are planning to travel to Bangalore (300kms)so that I can go for work from Monday .Doctor suggested no harm in travelling .but elder family members are suggesting not to travel .please give me your suggestion .I am really confused .

  128. Dear Doctor, your article is much appreciated and very helpful. I would like to request for your expertise and wisdom. I had a 3 day ET this past Monday. Out of 9 they fertilized and implanted 2. I am 37 and my husband is 15 years older than me. My doctor said the fertilized eggs are of good quality and good morphology. However, I just wonder why only 2 were fertilized out of 9. Apart from that, is it harmful to have 1 or 2 roll up cigarettes after ET? thank you for your time

    2. Meds used were: Gonal 250 (2 weeks) and Cetrotide (1 week) and Ovitrelle a day before egg collection. 9 grown and 2 fertilized (good quality). this is the info provided by the ivf centre. I do not know the endo thickness, but the doc told me it was good. same with the estradiol level.
      I much appreciate your response and do not want to take up any of your time. thank you for any enlightments you may have. if not enough info from my side, i apologize and I thank you anyway, sir.

    3. Where are the photos of your embryos ?

      This is not correct. You have a legal right to your medical records - every hospital has to provide them by law ! Please make a request for this in writing.

      The only tangible product an IVF clinic can deliver is embryos ! All good clinics provide embryo photos proactively and routinely

      Any clinic which does not provide embryo photos is a poor quality clinic.

      Please find a better clinic if you want to maximise your chances of success - one which does only blastocyst transfers and provides embryo photos routinely to all patients

    I have had a blasto two weeks back and my hcg test came out positive with 2076 as the level. I am really happy however I have this craving for having sex and I am too scared to ask my doctor whether we can maintain relations or not? Could you please assist?     1. Congrats ! Yes, it's safe to have sex - your embryos are safe in your uterus. Its quite common for libido to increase during pregnancy because of the increased pelvic congestion/ blood flow

    Age: 36
    2nd Frozen Embryo Transfer
    Day 5 Blastocyst Grade 1
    Quantity: 2

    1st Beta: 13dp5dt: 470.97
    2nd Beta: 15dp5dt: 1080.70
    Have been called for scan after 10 days from 1st beta.
    During beta time i had some morning sickness for 2-3 days but now its 17dp5dt and from past 2 days i have no symptoms at all. No nausea, no cramps nothing. Am scared if this is Anembryonic Preg. Am i getting paranoid? Is no symptom safe and common?

    Our 1st FET resulted in chemical preg. Doc said it can be because quality of embryo was not genetically good. If thats the case does it also mean this preg also will have complications? Since egg collected and transferred was/is from the same lot.
