What precautions should I take after the embryo transfer ?


One of the commonest questions patients ask me is - " Doctor, what are the dos and don'ts after the embryo transfer ? " They are very worried about the risk of failure, and are petrified that they may end up doing something which will cause their embryos to "fall out" of the uterus. Many will often spend the entire 2 week wait obsessing about their diet and physical activity.

Some will even put themselves on voluntary house arrest. Even worse, their relatives will often force them to sleep in bed for 14 days after the transfer. And to add insult to injury, many IVF doctors will also "advise" this !

Why do we still continue to perpetuate this myth ? I think it's partly doctors who are responsible For one thing, many doctors do not bother to educate their patients that physical activity will not affect embryo implantation. Others have a hidden agenda. Some want to hold on to their patients and by claiming that bed rest is essential after the IVF treatment, they can dissuade their patients from going to Bombay for their treatment ! Other doctors use this as a subtle way of " blaming the victim " if the cycle fails. ( " Your IVF cycle failed because you did not rest" is the hidden subtext !)

The reality is that bed rest does not help to improve pregnancy rates after IVF. Many studies have proven this conclusively. As I remind patients, the human body has been designed with enough sense, that embryos do not fall out if your cough or sneeze ! The truth is that human reproduction is inefficient and we cannot control the outcome. Whether the embryo implants or not is a biological process which the patient cannot influence in any way !

Am I taking an excessively casual approach ? No, I am just taking a realistic approach ! The problem is that when patients do not get pregnant, they often end up blaming themselves - and a common regret is - I did not rest enough. And even if they are smart enough not to do so, there are enough "friends and well-wishers" who will do this for them ! " What , you did not rest ? Your doctor must be crazy giving you such bad advise ! When my sister did her IVF in Delhi, her doctor made her rest for 9 months and now she has a beautiful 2 year old daughter" !

Once the embryos have been transferred into the uterus, they are safe and cannot "fall out " . This is simple commonsense ! IVF embryos , once they have reached the uterus, do not "remember" that they have spent 3 days in an IVF incubator - they are just like all other "in vivo" embryos which are formed after intercourse in the bedroom. Whether the embryo spends 3 days in your fallopian tube or in a test tube makes no difference to the outcome !

You need to have enough maturity to understand that the final outcome is not in your hands. It's important that you not blame yourself in case you do not get pregnant - doing this just makes a bad situation worse ! Trust your gut instinct and your commonsense - your body has enough wisdom to guide you as to what is fine and what is not. My advise is simple - Don't do anything which you may regret later !

Curtailing physical activity during the 2ww is a huge challenge for women who already have a baby and are doing IVF to complete their family. They feel torn between their desire to pick up their toddler and their fear that this "strain" may expel the embryo !

A word of advise to spouses. You are very likely to be overprotective during this time. You will want her to "rest" and "take things easy". You will tell her not to go to work, or to be very careful as to what she drinks or eats. While I am sure your wife will enjoy this pampering, please do not go over the top. She is smart enough to figure out what to do - after all, she is the one who has the most at stake, and is unlikely to do anything which will jeopardise her chances of getting pregnant ! Try to protect her from the well-meaning advise of busy-bodies, who are often very poorly informed !

Actually, this bed rest can be harmful; and we do our best to encourage our patients to go back to work after the embryo transfer ! Keeping yourself gainfully occupied is your best bet. Sitting at home when you are well can be very demoralising.

It can be very hard to deal with the uncertainty and anxiety of the 2 week wait ! The emotional stress is enormous - and lying in bed all day just exacerbates this a thousand fold ! Patients on bed rest tend to brood all day long about the outcome. Every minor twinge and ache is magnified, and your mind plays games with you during the 2 week wait - and commonsense suggests this stress cannot be good for you ! Moreover, when you are on bed rest, you are a burden to the rest of the family, and this adds to your guilt ! And if the cycle fails, then you end up blaming yourself ( I got up to pee after the embryo transfer, and this caused my embryos to get dislodged !)

You cannot influence the outcome - all you can do is gracefully let nature take its own course !

Sir even if some one doesn't take bed rest then what are the things one should avoid after ET?? I mean any specific precautions need to be taken after ET?

Sir, I really don't know u like the other here who see you as an authority. However, u made what people in my country refers to as "too much sense". I am Nigerian and I had my ET some days ago in my country, I was at work the next day and I just believe that God has made it very successful. I found ur advice easy to understand and very educating as well as helpful. I only wish dat more people will read until they get to this article and act on it. Thank you for the info. I really needed it this particular period of time. I had my FET transfer yesterday and I was not sure if I did something wrong last month that made the fresh transfer failed. Your article really helped me not to blame my self for the past and pray for God to help me that things would work for me this time....

Hiya dr, i just had icsi transfer today and I'm resting up I was a bit worried about going to work tomorrow, but now I'm more calm about it. And I'll remember to use my common sence as my job is worki g with those that carnt help them selves, so lots of hoisting lifting ect. But I'll stay well away from all than and just do light dutys. Your prayer was lovely and insightfull, I'll keep it with me always x

Dr. M I just had my FET done yesturday and the only thing I am stressing about is that I took ibuprofen 800mg 1x at 11 pm the night before. My transfer was at 9:45am. My RE didnt mention not to take it but when he said after not to take ibuprofen only Tylenol for cramps I said oh no I took some last night. He and the other doctor looked at each other and then were like I'm sure it's out of your system. But it looks like it stays in your system 24 hrs and can mess with implantation and early miscarriage.

No, that is NOT true. Think about this for a minute. Women have sex during their fertile time, without even knowing they are ovulating. Many take Ibuprofen before and after ovulation, and \ does not have any effect on their fertility at all.

Please stop beating up on yourself !

There are no special precautions you need to take. Implantation is a natural biological process !

Sir, I hav two questios:
1. If vaginal progesteron 8% gel is a very good option why it is not being prescribed by most practioners?
(It is a traumatising experience to take the injectable progesterone. cant bear them!!!)

2. what is the role of Prednisolone after embryo transfer because I have been told to take it.

We stopped using injectible progesterone 20 years ago !

Prednisolone is a steroid and some doctors believe it helps embryo implantation ( by suppressing the immune system)

I got bleeding on 12 th day post ET.(it was the day due for my period) my UPT is Also Neg. so it means implantation didnt occur?

Is it advisable to go for FET in the coming cycle itself?

A negative UPT is not a good sign :(
However, you should do a blood test for HCG as well - this is much more reliable.

Yes, you can do a FET in the next cycle if you have bounced back

Dr pls help,after my normal menses on the days that i am susposed to be ovulating ,i start supporting. This have been going on since october 2012, i did pap smear test,my Dr said the result was ok. I did hormonal profile,and all they could see was that i am not ovulating. Pls what do you think is wrong wit me. I want to conceive.

This maybe because you have PCOD ( polycystic ovarian disease).

You need the following medical tests.
1. blood tests for you for the following reproductive hormones - FSH ( follicle-stimulating hormone),LH ( luteinising hormone),PRL ( prolactin) , AMH ( anti-Mullerian hormone) and TSH ( thyroid stimulating hormone)
( to check the quality of your eggs). Do this from a reliable lab such as SRL ( www.srl.in);
2. a vaginal ultrasound scan which should check for the following.

a. ovarian volume
b. antral follicle count
c. uterus morphology
d. endometrial thickness and texture

Please send me the detailed test results and medical reports . You can scan them in as a single Word file and email them to me.

Once we know what the problem is, then we can treat it !

Hi Dr, I had a day 3 ET on Monday and did acupuncture before& after and again on Thursday ( day 6). The acupuncturist was using heat packs to keep me warm during treatment. Is this safe? He insisted that it was an important part of the treatment as it helped to increase blood flow to the uterus & this is the main aim of acupuncture after transfer but I was worried that it may overheat the embryo. He also used gentle massage on my stomach and more vigorous on my lower back & again said it was safe for the embryo but I have been worried about it ever since and have many sharp twinges in my abdomen, some just from sitting up. Thanks for your help.

Hi dr i had my ivf 3 days ago and i felt orgasm on the third day can that harm ? And during my firs two hours i sneezed 2 times also can that harm?? Thank you one more question does travellin in a car during the first 14 days can affect the result?

my wife is normal but i have vas absent
I had done my ivf treatment with tesa

I had done my et on 2nd day with 3 embryo
1 is 4 cell
2 and 3 is 2 cell
So sir i want to know that it is a normal level of embryo on 2nd day?
My wife age is 24 yrs how many chansses to get result in positive

Dear Dr. I had ET 3 days around 8 days ago with 8 cell grace 1. Now still waiting I have worry a little bit. Can I do urine test on 2 more days

Too early to do a urine test for 2 reasons.

1. Even if you are pregnant, the urine test may not be positive, because the HCG levels in the urine are often very low at this time. This is called a false negative, and can depress you.

2. Even if you are not pregnant, the urine test may be positive, because of the HCG present in your body as a result of the HCG trigger shot given before egg collection. This is called a false positive, and can create false hope.

Please be patient - I know this waiting can be hard !

sir: doctor suggest us ICSI . They retrived 4 egg , 3 fertilise and 3 embryo of grade 1 was transfer doctor said that the grade of transfer is also 1 .I have no more knowledge about this pls help me that there are how many chances of get pregnancy.

Hi.. tried placing this somewhere else on this site as well but didnt happen. My question was that i had to get up immediately after my embryo transfer as my bladder was too full although i laid down for half an hour after that. do i need to worry considering i have had two ectopics before. been worried sick about this?
